  • Results from a home based exercise protocol to elderly people with osteoarthritis

    Introduction: Osteoarthritis is one of the most commom problems in elderly populations and the most commom cause of disability. Knee is one of the most affected joint and the loss of range of motion and function will limit daily activities and the mobility. Self-management programs with exercises performed at home may be a useful therapy. These programs are based on the quadriceps strengthening exercises, low aerobic strength in order to improve the functional state, pain and aerobic capacity. Objectives: Evaluate the effectiveness of an exercise protocol performed at home on improving the functionality, mobility and health related quality of life of elderly people with knee osteoarthritis. Materials and Methods: Exploratory, descriptive and longitudinal study. 44 subjects have been selected according to the inclusion criteria (age ¡Ý 65 years with symptoms of knee osteoarthritis) and exclusion (inability to perform gait, hip osteoarthritis and prosthetics hip or knee). The subjects made an initial assessment (T0) before beginning the protocol (applied during 8 weeks, daily, in the summer of 2013) performed at home and meeting occasionally in groups, with the researcher, to correct the learned exercises, clarify questions and add some exercises or progressions. After the 8 weeks was applied an new assessment (T1) and the follow-up is taken in T2 and T3, (respectively, at 3 and 6 months later). In the assessment was used the Knee Injury and Osteoarthritis Outcome Score (KOOS), the Timed Up and Go Test (TUG) and the SF-12. Data from changes in daily routine of the subjects are also collected. The protocol was applied by two physical therapists. Results: Between T0 and T1 the subjects present better results in the analyzed variables. Between T1 and T2, these improvement appear be related with the maintenance of the protocol. Conclusion: An home based exercise protocol to elderly people with osteoarthritis revealed results in mobility. The maintenance of the results is related with the continuity of the protocol and with changes in the daily activities.
  • Prevenção de lesões em estudantes de música : intervenção da fisioterapia

    Os músicos e estudantes de música apresentam inúmeras queixas músculo-esqueléticas, tais como dor e fadiga muscular associadas à prática de instrumentos. Estas podem dever-se à intensidade e exigência da técnica e a posturas repetidas e mantidas. Não havendo uma intervenção terapêutica ou uma mudança de hábitos, o músico poderá perder alguma da sua funcionalidade, desenvolver assimetrias posturais, perder força e endurance, sen􀆟r dor e desconforto. Inicialmente foram distribuídos 2 ques􀆟onários (Nulli 20- Índice do Pescoço e Membro Superior e um ques􀆟onário realizado pela autora do estudo) a 41 alunos do curso de música. Na aplicação prática do estudo, foram realizadas sessões de ensino com o aconselhamento de estratégias que contribuem para a diminuição de queixas e prevenção de lesões; classes de movimento (7 sessões); sessões de alongamentos (2 sessões) e sessões de relaxamento (2 sessões). As queixas nos alunos de música centram-se na região cervical, lombar e nos membros superiores, nomeadamente nos ombros e punhos. Na sua grande maioria, foi referida a presença de formigueiros nos membros superiores. Rela􀆟vamente às sessões prá􀆟cas, verificaram-se melhorias ao nível das atividades fisicas, do impacto na vida psicossocial e no sono, embora sem significado estatistico. Não se verificaram alterações ao nível do trabalho e relativamente aos efeitos iatrogénicos houve um aumento dos sintomas. No entanto, a pontuação total dos ques􀆟onários revela melhorias para os sujeitos que 􀆟veram intervenção da fisioterapia. Conclusão: A criação de programas de prevenção de lesões nas escolas de música e nos conservatórios pode ser benéfica, contribuindo para a diminuição das queixas já existentes e para a prevenção de lesões, aumentando desta forma a qualidade de vida e a eficácia destes profissionais.
  • Um projeto de educação clínica para pessoas com doença pulmonar obstrutiva crónica (DPOC)

    Introduction: The WHO estimates that COPD reaches 210 million people worldwide. Patients with COPD must have access to adequate information about their health condition, making the active participation in the management and treatment of their illness as possible. One project was developed in clinical education of the physiotherapy undergraduate course, to assess the impact of a home program. Objective: To evaluate the results of the intervention at home (educational approach). To enable the patient to deal with his health condition and make it autonomous in the control of COPD. Methods: A home program with 10 COPD patients was performed. The program was divided into 8 separate weekly sessions. The evaluation occurred before - and after the intervention (T0-T1). A form was applied to collect demographic and clinical data, the Modified British Medical Research Council Questionnaire, COPD Assessment Test and the London Chest ADL’s. Gas analyses were also carried out. The statistical analysis was performed using SPSS, using the comparison of means and the non-parametric Wilcoxon test. Results: The mean scores of the MMRC, CAT and LCADL decreased from. However, only significant differences in relation to MMRC (p=0.021) and LCADL (p=0.014). Regarding the data obtained by gas analysis the average SaO2, pO2 and FO2Hb increased from T0 to T1, but none was significant. Conclusions: After domiciliary intervention patients with COPD felt especially improvements in functional terms and symptomatology. The results from blood gas analysis, are not sufficient to conclude that the intervention have a metabolic level results.
