Abstract(s)
O presente relatório tem como principal objetivo o de descrever o percurso
realizado ao longo da prática clínica caracterizada pela realização de um estágio
profissional numa equipa de cuidados paliativos e pela implementação de um projeto
de intervenção no local de trabalho.
As atividades desenvolvidas centram-se nos cuidados paliativos na vertente
hospitalar especificamente no serviço de urgência, sendo este o tema principal deste
trabalho. O estágio realizou-se numa Equipa Intra-Hospitalar de Suporte em Cuidados
Paliativos num hospital central e universitário. Ao longo de cinco semanas adquiriu-se
competências no atendimento e acompanhamento às pessoas em situação paliativa
e suas famílias, tendo por base os valores e princípios desta filosofia de cuidados. Para
demonstrar competências na implementação de um plano assistencial de qualidade
realizou-se um estudo de caso. Face às necessidades da equipa, elaborou-se um
documento de apoio e acompanhamento à pessoa em situação paliativa,
sistematizando registos de toda a informação relativa à gestão terapêutica em
documento, sob a forma de boletim, ao doente e família. Mediante o trabalho efetuado
no decorrer do mestrado, integrou-se no programa de formação da Equipa Intra-
Hospitalar de Cuidados Paliativos, a apresentação e partilha de saberes de uma
revisão sistemática da literatura alusiva às dificuldades dos enfermeiros do serviço de
urgência na prestação de cuidados paliativos.
O projeto de intervenção: Cuidados Paliativos em Serviço de Urgência consistiu
essencialmente na sensibilização dos diversos profissionais para a abordagem e ações
paliativas em doentes crónicos, com doença avançada que chegam ao serviço de
urgência, onde desempenha a sua atividade profissional. A formação de um grupo
dinamizador para os cuidados paliativos foi necessária para assegurar a continuidade
deste projeto.
O crescimento pessoal e profissional foram fatores de motivação ao longo do
processo formativo onde a aquisição de competências nas diferentes áreas chave dos
cuidados paliativos foi determinante.
ABSTRACT: The present report has a main objective to describe the whole process carried on during the clinical practice characterized by completion of an internship in a palliative care team, and the implementation of an intervention project in the workplace. The activities developed were focused on palliative care in hospital setting, more specific at the emergency department, which was the core theme of this work. The internship was performed at Inpatient Support Team in Palliative Care of University Hospital. After five weeks were acquired skills in the care and follow up of patients that are in palliative conditions and their families. These skills were based on the values and principles of palliative care. It was developed a case-study with objective to demonstrate the skills in the implementation of healthcare plan with quality. In according with needs of palliative team, was drew up a document to follow up and support patient in the palliative conditions. Registers of all information on therapeutic management were recorded of a systematized way, in format of booklet and it was given to patient and family. Beyond of work done during the Master, a presentation and share of knowledge were integrated in a systematic literature review demonstrating all difficulties of Nurses from emergency department in provide palliative care. The project Palliative Care in Emergency Room essentially consisted in give awareness about approach to palliative and chronically ill patients with advanced disease, at the emergency room, where Nurses plays their professional activity. The composition of a facilitator group for palliative care was needed to ensure the continuity of this project. The personal and professional growth were motivating factors of this training process, where the acquisition of skills in different key areas of palliative care was crucial.
