Integrado no Mestrado em Cuidados Paliativos da Escola Superior de SaĂșde Dr.
Lopes Dias do Instituto Politécnico de Castelo Branco, serve o presente relatório para
descrever, relatar e reflectir, crĂtica e pormenorizadamente, sobre todas as
actividades preconizadas em Projecto de PrĂĄtica ClĂnica e RelatĂłrio e realizadas na
Unidade Curricular de PrĂĄtica ClĂnica.
A PrĂĄtica ClĂnica foi efectuada numa Unidade de Cuidados Continuados e
Paliativos em contexto hospitalar privado, no total de 300 horas, contabilizando-se 37
turnos. Cruzando a observação directa e participante com a imersão na mais actual
evidĂȘncia cientĂfica, descreve-se o modelo de organização e funcionamento da equipa
de Cuidados Paliativos, aprofundando a tipologia de prestação de cuidados e
caracterĂsticas do serviço; reflectem-se as competĂȘncias adquiridas nas ĂĄreas-chave
dos Cuidados Paliativos, especificamente no trabalho em equipa, comunicação,
controlo de sintomas e apoio Ă famĂlia; e, demonstra-se a aquisição de competĂȘncias
na estruturação e implementação de programas em Cuidados Paliativos.
Como temåtica do projecto de intervenção implementado na Unidade de Cuidados
Paliativos, seleccionou-se a realização de conferĂȘncias familiares. Conforme a filosofia
do cuidar em Cuidados Paliativos, a qualidade dos cuidados revĂȘ-se na assistĂȘncia
prestada ao doente, bem como Ă sua famĂlia, sendo esta uma atenção necessĂĄria e
indissociåvel para a promoção da qualidade de vida de ambos os elementos e
prevenção e alĂvio do seu sofrimento. Neste Ăąmbito, surge como ferramenta essencial
e diferenciadora a conferĂȘncia familiar, considerada um importante instrumento de
trabalho dos profissionais de saĂșde para apoiar as famĂlias e maximizar o sucesso das
suas intervençÔes.
ExpÔe-se o planeamento, desenvolvimento e avaliação do projecto aplicado na
Unidade de Cuidados Paliativos, baseado no estudo do indicador de qualidade da
realização de conferĂȘncias familiares em Cuidados Paliativos proposto pelo Doutor
Manuel LuĂs Vila Capelas, Presidente da Associação Portuguesa de Cuidados
Paliativos.
Este relatĂłrio sublinha a realização de conferĂȘncias familiares como intervenção
necessĂĄria e eficaz em Cuidados Paliativos e promove o desenvolvimento teĂłrico-prĂĄtico
do tema. Identificam-se as conferĂȘncias familiares como ĂĄrea de melhoria da
qualidade na Unidade de Cuidados Paliativos e apresenta-se uma proposta de
melhoria da qualidade ao respectivo serviço.
Através da acção de formação à equipa multidisciplinar e da elaboração de um
algoritmo de intervenção das conferĂȘncias familiares em Cuidados Paliativos,
contribuiu-se para melhorar a capacidade e a qualidade das conferĂȘncias familiares
enquanto intervenção em Cuidados Paliativos.
ABSTRACT: As part of the masterâs degree in Palliative Care from the Escola Superior de SaĂșde Dr. Lopes Dias, Instituto PolitĂ©cnico de Castelo Branco, this document aims to report, describe and reflect, in a critical and detailed way, about the activities recommended in the Clinical Practice Project and held in the Clinical Practice curricular unit. The Clinical Practice was performed in a Palliative Care Unit, part of a private hospital, totaling 300 hours over the span of 37 shifts. Crossing direct and active observation with the most state-of-the-art scientific evidence, the organization and working model of a Palliative Care team is described, especially regarding care provision type and service traits; a reflection is made about the competences acquired in Palliative Care key areas, namely teamwork, communication, symptom control and family support; and proof is given of the acquisition of competences on the structuration and implementation of Palliative Care programs. The subject chosen for the intervention project implemented in the Palliative Care Unit was the family meeting. As part of the Palliative Care philosophy, the quality of the care can be measured by the assistance given to the patient as well as to his/her family, this being essential to promote the quality of life of both elements and the prevention and relief of their suffering. In this context, the family meeting emerges as an essential and distinguishing tool, seen as an important work instrument of the health care professionals in the support of the families and maximization of the success of their interventions. The planning, development and evaluation of the project executed in the Palliative Care Unit is exposed, taken in consideration the study of the quality index of the family meeting in Palliative Care, proposed by Doctor Manuel LuĂs Vila Capelas, head of the Associação Portuguesa de Cuidados Paliativos. This report highlights the execution of family meetings as a needed and effective intervention in Palliative Care and promotes the development of the theoretical and practical knowledge on the subject. The family meetings are identified as an improvement area of the quality in the Palliative Care Unit and a proposal for improvement is made. By means of a training course and the elaboration of an algorithm for the family meeting in Palliative Care, a contribute was made to improve the ability and quality of the family meeting as part of the Palliative Care intervention.
