O presente Relatório é parte integrante dos requisitos para obtenção do grau de
Mestre no âmbito do 8º Mestrado em Cuidados Paliativos da Escola Superior de
Saúde Dr. Lopes Dias do Instituto Politécnico de Castelo Branco.
Este relatório é composto por três partes. A primeira é uma descrição analítica,
crítica e reflexiva, sustentada por referências científicas e ilustrada por exemplos
clínicos, de um período de prática clínica junto de uma equipa diferenciada de
cuidados paliativos, através dum estágio que foi observante e participativo.
As duas partes seguintes são dirigidas ao tema dos cuidados paliativos na pessoa
com esquizofrenia e doença física, fazendo a junção com a nossa especialidade, a
psiquiatria. As pessoas com esquizofrenia são uma população particularmente frágil
conhecida por carecer de cuidados de saúde físicos, inclusivamente de cuidados
paliativos. Estes doentes recebem menos e piores cuidados paliativos e apresentam
desafios clínicos particulares.
Na segunda parte apresentamos um projeto de intervenção nos dois serviços em
que trabalhamos, o Departamento de Psiquiatria e Saúde Mental e a Equipa Intrahospitalar
de Suporte em Cuidados Paliativos, do Hospital do Espírito Santo de Évora,
E.P.E. Este projeto foi a criação de uma checklist de boas práticas para ser usada nesta
população de doentes, para garantir a provisão ótima de cuidados e evitar os
obstáculos inerentes a este contexto clínico.
Na terceira parte apresentamos uma revisão sistemática da literatura sobre o
planeamento antecipado de cuidados paliativos em pessoas com esquizofrenia, sendo
que o planeamento antecipado de cuidados representa um elemento fundamental na
prestação ótima de cuidados.
ABSTRACT: This Report is an integral part of the requirements for obtaining a Master’s degree as part of the 8th Master's Degree programme in Palliative Care of the Dr. Lopes Dias College of Health at the Polytechnic Institute of Castelo Branco. This report is composed of three parts. The first is an analytical, critical and reflective description, supported by scientific references and illustrated by clinical examples, of a period of clinical practice with a specialist palliative care team, during an internship that was both observant and participative. The following two parts deal with the theme of palliative care in the person with schizophrenia and physical illness, establishing a link with our specialty, psychiatry. People with schizophrenia are a particularly fragile population with unmet physical health care needs, including palliative care. These patients receive less and inferior palliative care and present specific clinical challenges. In the second part, we present an intervention project for the two health services in which we work, the Department of Psychiatry and Mental Health and the Intrahospital Support Team in Palliative Care, of the Hospital do Espírito Santo de Évora, E.P.E. This project was the creation of a checklist of good practices to be used in this patient population, to ensure optimal care provision and to avoid the obstacles inherent to this clinical context. In the third part, we present a systematic review of the literature on advance planning in palliative care for people with schizophrenia, given that early care planning is a key element in the optimal provision of care.
