O presente Relatório é parte integrante do 8º Mestrado em Cuidados Paliativos da Escola
Superior de Saúde Dr. Lopes Dias, tendo como objetivo principal refletir sobre os
conhecimentos apreendidos ao longo da aprendizagem teórica e consolidar as competências
adquiridas durante a prática assistencial de Enfermagem à pessoa em situação crónica e
paliativa.
A prática clínica foi desenvolvida numa Equipa Intra-Hospitalar de Suporte em Cuidados
Paliativos e Equipa Comunitária de Suporte em Cuidados Paliativos. Explanei o tema
“Intervenção do enfermeiro na capacitação do cuidador informal para cuidar do doente em
últimos dias e horas de vida no contexto domiciliário”, tendo realizado uma revisão
sistemática da literatura, cujos objetivos foram averiguar como se pode capacitar o cuidador
para cuidar do doente nos últimos dias e horas de vida em contexto domiciliário e determinar
qual a importância dessa capacitação, que serviu de mote para o Projeto de Intervenção -
Intervenção do enfermeiro na capacitação do cuidador informal para cuidar do doente em
últimos dias e horas de vida no contexto domiciliário. Neste âmbito realizei um estudo com o
objetivo de conhecer as necessidades sentidas pelos cuidadores informais para cuidar do
doente em últimos dias e horas de vida no domicílio. Recorri a um inquérito por questionário
realizado para o efeito, aplicado numa amostra de 15 cuidadores informais de doentes
acompanhados pela Equipa Comunitária de Suporte em Cuidados Paliativos. Os principais
resultados revelam que a maioria dos participantes respondeu ter “Médias”, “Muitas” e
“Bastantes” dificuldades para cuidar do doente em últimos dias e horas de vida no domicílio.
Com base no diagnóstico das dificuldades sentidas, foi elaborado um guia de apoio ao
cuidador informal, como forma de os apoiar/capacitar para cuidar do doente em últimos dias
e horas de vida no domicílio. Foi também efetuada uma formação a elementos da Equipa de
Cuidados Paliativos e aos elos de ligação de Cuidados Paliativos dos outros serviços, cujos
objetivos consistiram em reconhecer a importância de identificar o local de preferência de
morte do doente e da necessidade de capacitar o cuidador informal para cuidar do doente em
últimos dias e horas de vida no domicílio.
This Report is an integral part of the 8th Master's Degree in Palliative Care at the Escola Superior de Saúde Dr. Lopes Dias, with the main objective of reflecting on the knowledge learned during theoretical learning and consolidating the skills acquired during the nursing care practice for the person in chronic and palliative situation. The clinical practice was developed by an In-Hospital Palliative Care Support Team and a Community Palliative Care Support Team. I explained the topic “Nurse's intervention in training informal caregivers to care for the patient in the last days and hours of life in the home context”, having carried out a systematic review of the literature, whose objectives were to ascertain how the caregiver can be trained to care for the patient in the last days and hours of life in the home context and determine the importance of this training, which served as a motto for the Intervention Project - Nurse's intervention in training the informal caregiver to care for the patient in the last days and hours of life in the home context . In this context, I conducted a study with the objective of knowing the needs felt by informal caregivers to care for the patient in the last days and hours of life at home. I used a questionnaire survey carried out for this purpose, applied to a sample of 15 informal caregivers of patients followed by the Community Support Team in Palliative Care. The main results reveal that the majority of the participants answered that they had “Mediums”, “Many” and “Enough” difficulties in caring for the patient in the last days and hours of life at home. Based on the diagnosis of the difficulties experienced, a guide to support the informal caregiver was developed as a way to support/enable them to care for the patient in the last days and hours of life at home. Training was also carried out on elements of the Palliative Care Team and on the Palliative Care links of other services, whose objectives were to recognize the importance of identifying the patient's preferred place of death and the need to train the informal caregiver to care for the patient in the last days and hours of life at home.
