O presente RelatĂłrio insere-se no Ăąmbito do 8Âș Mestrado em Cuidados Paliativos da Escola Superior de SaĂșde Dr. Lopes Dias, do Instituto PolitĂ©cnico de Castelo Branco e Ă© parte integrante dos requisitos para obtenção de grau. Este relatĂłrio encontra-se dividido em duas partes e pretende refletir sobre os conhecimentos e competĂȘncias adquiridos durante o primeiro ano de aprendizagem teĂłrica e consolidados durante a prĂĄtica clĂnica. A primeira parte Ă© uma descrição crĂtica e reflexiva sobre a prĂĄtica clĂnica e divide-se
em dois capĂtulos. O primeiro capĂtulo Ă© dirigido Ă caracterização do contexto de estĂĄgio, com descrição do serviço, sua missĂŁo e equipa, onde se inclui ainda uma breve anĂĄlise estatĂstica dos casos acompanhados. O segundo capĂtulo apresenta uma reflexĂŁo crĂtica sobre as competĂȘncias adquiridas e a concretização dos objetivos delineados atravĂ©s da discussĂŁo dos casos clĂnicos mais desafiantes e marcantes. A segunda parte Ă© dedicada ao projeto de intervenção, que se centra na ĂĄrea dos cuidados em fim de vida, nomeadamente nas Ășltimas horas e dias de vida, e que surgiu naturalmente uma vez que a prestação de cuidados de saĂșde de excelĂȘncia a doentes em fim de vida foi o mote para a concretização deste Mestrado. Neste sentido, foi
desenvolvida uma ação de formação nesta ĂĄrea do cuidar com o propĂłsito de sensibilizar e dotar os profissionais de saĂșde com competĂȘncias e conhecimentos que permitam um melhor acompanhamento a estes doentes. Nesta parte apresenta-se tambĂ©m uma revisĂŁo sistemĂĄtica da literatura sobre a vivĂȘncia da solidĂŁo no fim de vida.
Abstract: This report falls within the scope of the 8th edition of the Masterâs Degree in Palliative Care of the Escola Superior de SaĂșde Dr. Lopes Dias, of the Instituto PolitĂ©cnico de Castelo Branco and is a part of the requirements for obtaining the degree. The report is composed of two parts and aims to reflect on the knowledge and skills acquired during the first year of theoretical learning and strengthened during clinical practice. The first part consists of a critical and reflexive description of the clinical practice and is divided into two chapters. The first chapter focuses on characterizing the context of the internship, providing the unitâs description, its mission and the team, including a brief analysis of the clinical cases attended. The second chapter presents a critical reflection on the acquired skills and the accomplishment of the outlined goals through the discussion of the most challenging and remarkable clinical cases. The second part, stemming from this Masterâs degreeâs purpose of further understanding how to provide exceptional health care to terminal patients, is dedicated to the intervention project, centered on end-of-life care, particularly in the last days and hours of life. In this regard, a training course on the topic was developed to sensitize and endow health care professionals with the skills and knowledge to provide better care in the last hours and days of life. This part also presents a systematic review of the literature on experiencing loneliness at the end of life.
