Este documento foi elaborado tendo como objetivo a obtenção de grau de Mestre
pela Escola Superior de Saúde Dr. Lopes Dias do Instituto Politécnico de Castelo
Branco, no âmbito do mestrado de Cuidados Paliativos. Este relatório encontra-se
dividido em três capítulos.
O primeiro capítulo corresponde à Prática Clínica, onde é descrito o estágio de 300
horas realizado numa Equipa de Cuidados Paliativos. Durante o período de estágio
foram observados um total de 98 doentes, 51 em regime de internamento, 38 em
consulta externa e 9 doentes no contexto da Equipa Domiciliária de Suporte em
Cuidados Paliativos.
O segundo capítulo descreve as competências adquiridas no âmbito dos pilares
dos Cuidados Paliativos (Comunicação, Controlo de Sintomas, Apoio à família,
Trabalho de equipa), com a análise das principais competências e objetivos
delineados para a Prática Clínica e a reflexão de um caso clínico.
O terceiro capítulo apresenta o Projeto de Investigação e Formação que serve o
principal propósito de melhoria da qualidade dos cuidados à família e ao doente com
necessidades paliativas. Este capítulo engloba a Revisão Sistemática da Literatura
sobre as necessidades formativas do cuidador informal do doente com necessidades
paliativas, no domicílio.
No decurso deste relatório está em enfoque a abordagem individualizada e
holística, numa perspetiva de cuidados longitudinais do Médico de Família que presta
cuidados de proximidade e ocupa uma posição privilegiada no acompanhamento do
doente-família, nomeadamente no que respeita à prestação de Cuidados Paliativos.
Estando ainda longe de um padrão ideal da integração dos Cuidados Paliativos nos
Cuidados de Saúde Primários, é imperativo ético a priorização de uma medicina
personalizada e integradora, centrado no doente e família.
Abstract : This document was developed with the aim of obtaining a Master's degree from the Escola Superior de Saúde Dr. Lopes Dias of the Instituto Politécnico de Castelo Branco, in the area of Palliative Care. This report is divided into three chapters. The first chapter deals with Clinical Practice, describing the 300-hour internship in a Palliative Care Team. A total of 98 patients were seen during the internship, 51 in hospital, 38 in consultations and 9 in the context of the Palliative Care Home Support Team. The second chapter describes the skills acquired within the framework of the pillars of Palliative Care (Communication, Symptom Control, Family Support, Teamwork), analysing the main competences and objectives for Clinical Practice and reflecting on a clinical case. The third chapter presents the Investigation and Training Project which serves the main purpose of improving the quality of care for the family and the patient with palliative needs. This chapter includes the Systematic Literature Review on the formative needs of the informal carer of a patient with palliative needs at home. Throughout this report, the focus is on the individualised and holistic approach, from a longitudinal care perspective, of the Family Doctor who provides proximity care and occupies a privileged position in accompanying the patient and family, particularly with regard to the provision of palliative care. Although we are still far from an ideal standard for integrating Palliative Care into Primary Health Care, it is an ethical imperative to prioritise personalised and integrative medicine, centred on the patient and family.
