O presente relatório de estágio insere-se no âmbito do 10.º Mestrado em Cuidados Paliativos da Escola Superior de Saúde Dr. Lopes Dias do Instituto Politécnico de Castelo Branco (IPCB). Este documento tem como finalidade a reflexão da prática clínica, a demonstração das competências adquiridas nas áreas-chave dos Cuidados Paliativos e a descrição das atividades desenvolvidas no estágio e no projeto de intervenção/plano de formação. Na prestação de cuidados de saúde, os Cuidados de Saúde Primários (CSP) têm um papel fundamental no acompanhamento da população na sua vertente biopsicossocial, desde o nascimento até à morte. Na fase da doença progressiva, crónica e incurável, a integração com os Cuidados Paliativos é fundamental para providenciar os melhores cuidados de saúde aos utentes e às suas famílias. No entanto, a realidade do país varia de região para região, sendo muitas vezes difícil o acesso a estes cuidados especializados, principalmente em situações de doença não-oncológica. A parte prática do estágio decorreu na Equipa Comunitária de Suporte em Cuidados Paliativos na Unidade Local de Saúde X, durante os meses de maio e junho de 2022, tendo completado 350 horas de estágio nesse período. A reflexão crítica foi sustentada através da apresentação de um caso clínico, demonstrando competências adquiridas e os objetivos cumpridos através de todo o percurso curricular. O tema apresentado como revisão sistemática teve por base estudos que descreviam as principais barreiras à referenciação dos doentes não-oncológicos para os Cuidados Paliativos, sendo um problema transversal aos profissionais de saúde, aos doentes, aos familiares, às próprias instituições de saúde, assim como à própria comunidade. Torna-se assim imperativo mudar as mentalidades e quebrar as diversas barreiras à referenciação destes utentes, de modo a que possam ter acesso aos cuidados mais diferenciados de forma mais precoce.
Abstract: This internship report is part of the 10th Master's Degree in Palliative Care at the Escola Superior de Saúde Dr. Lopes Dias of the Instituto Politécnico de Castelo Branco (IPCB). This document aims to reflect on clinical practice, demonstrate the skills acquired in the key areas of Palliative Care and describe the activities developed in the internship and in the intervention project/training plan. In the provision of health care, Primary Health Care (PHC) plays a fundamental role in monitoring the population in its biopsychosocial aspect, from birth to death. In the phase of progressive, chronic and incurable disease, integration with Palliative Care is essential to provide the best healthcare to users and their families. However, the reality of the country varies from region to region, and access to this specialized care is often difficult, especially in situations of non-oncological disease. The practical part of the internship took place in the Community Palliative Care Support Team at Local Health Unit X, during the months of May and June 2022, having completed 350 hours of internship during this period. Critical reflection was supported through the presentation of a clinical case, demonstrating skills acquired and objectives achieved throughout the curricular path. The theme presented as a systematic review was based on studies that described the main barriers to the referral of non-oncology patients to Palliative Care, being a transversal problem for health professionals, patients, family members, health institutions themselves, as well as to the community itself. It is therefore imperative to change mentalities and break down the various barriers to referral of these users, so that they can have access to more differentiated care earlier.
