O presente relatório é realizado no âmbito do 8º Mestrado em Cuidados Paliativos
da Escola Superior de Saúde Dr. Lopes Dias (ESALD), do Instituto Politécnico de Castelo
Branco (IPCB), como parte integrante dos requisitos necessários à obtenção do grau de
Mestre. Assim, pretende-se integrar os conhecimentos adquiridos ao longo da
aprendizagem teórica, de forma a mobilizá-los na prática clínica e assistencial junto de
uma equipa diferenciada em CP, com o intuito de consolidar competências
instrumentais, interpessoais e sistémicas aplicando os valores e princípios dos CP.
Este documento irá constituir-se por três partes. Na primeira parte, abordam-se os
valores e princípios dos CP. É também realizada uma reflexão crítica acerca da tipologia
e modelo de organização do serviço de CP onde decorreu a prática clínica, bem como,
por uma descrição do processo de implementação e desenvolvimento do projeto de
intervenção.
Na segunda parte é realizada a reflexão crítica das atividades desenvolvidas em
articulação com os objetivos estabelecidos e as competências do enfermeiro. É
realizado um relato prático com base nos quatro pilares dos Cuidados Paliativos
(comunicação, controlo de sintomas, cuidar a família e trabalho em equipa).
A terceira parte deste documento descreve o projeto de intervenção/formação que
é dedicado ao tema do controlo sintomático da dispneia em últimos dias e horas de
vida. A dispneia é um sintoma altamente incapacitante e capaz de interferir
negativamente na qualidade de vida do doente em últimos dias e horas de vida. Assim,
justifica-se a importância de se conhecer as estratégias de intervenção adequadas para
cada situação. Por fim apresenta-se a revisão integrativa da literatura "Estratégias de controlo da dispneia nos últimos dias e horas de vida".
Abstract: This report is designed as part of the 8th Master's Degree in Palliative Care of the Escola Superior de Saúde Dr. Lopes Dias (ESALD), of the Instituto Politécnico de Castelo Branco, as an integral part of the requirements necessary to obtain a Master's degree. The aim of this course is to combine the knowledge acquired during the theoretical learning, integrated and mobilized in clinical practice and assistance with a team differentiated in CP, with the purpose of building and consolidating instrumental, interpersonal and systemic competences applying the values and principles of palliative care. It consists of three parts. In the first part, the values and principles of palliative care are discussed. A critical reflection is realized about clinical practice service tipology and organization model and a description of implemention and desenvelopment intervention project. In the second part, a critical reflection of the activities developed in articulation with the established objectives and the nurses' competences is presented. A practical report is carried out based on the four pillars of Palliative Care (communication, symptom control, family care and teamwork). The third part of this document describes the intervention/training project and is dedicated to dyspnea management in the last days and hours of life. The dyspnea is a crippling symptom and negatively interferes with the patient's quality of life. Thus the importance of knowing the appropriate intervention strategies for each situation is justified. Finally an intensive review of the literature "Strategies to control dyspnea in the last days and hours of life" is presented.
