O presente relatĂłrio surge, como parte integrante dos requisitos necessĂĄrios Ă obtenção do grau de Mestre em Cuidados Paliativos (CP). Ă um processo reflexivo relativo Ă prĂĄtica clĂnica desenvolvida junto de uma equipa diferenciada em CP, e Ă© orientado pela aquisição de competĂȘncias. De forma descritiva e ponderada, este relatĂłrio encontra-se suportado por evidĂȘncia cientĂfica, descreve e explica as atividades desenvolvidas e os objetivos pretendidos. Genericamente, encontra-se organizado em trĂȘs momentos: A prĂĄtica clĂnica desenvolvida, organizada segundo os pilares dos cuidados paliativos, indicando as competĂȘncias na implementação de um plano assistencial de qualidade Ă pessoa com doença crĂłnica, avançada e progressiva e Ă sua famĂlia, maximizando a qualidade de vida e diminuindo o sofrimento; apresentando a metodologia de aquisição de competĂȘncias instrumentais, interpessoais e sistĂ©micas. O projeto de intervenção implementado, com enfoque no diagnĂłstico clĂnico de Ăltimos Dias ou Horas de Vida (UDHV) que consistiu na elaboração de um plano de formação, apresentado Ă UCSP, onde desenvolvo a atividade profissional, capacitando os profissionais para o desenvolvimento de açÔes bĂĄsicas paliativas, identificação e referenciação precoce do doente com necessidades paliativas, particularizando para o perĂodo dos UDHV. E, no desĂgnio de aprofundar o conhecimento nesta ĂĄrea temĂĄtica tĂŁo vasta o desenvolvimento da RevisĂŁo SistemĂĄtica da Literatura (RSL) â âBenefĂcios da identificação precoce da fase de UDHV para o doente e sua famĂliaâ, tendo como principal objetivo identificar as principais consequĂȘncias no doente terminal e sua famĂlia, que resultam do diagnĂłstico precoce do diagnĂłstico de UDHV por parte dos Profissionais de SaĂșde (PS). Os UDHV constituem um perĂodo da vida, em que a morte estĂĄ eminente, e Ă© necessĂĄrio redefinir objetivos com o doente e respetiva famĂlia, exigindo-se nesta fase, cuidados especĂficos e adequados Ă s necessidades da pessoa, direcionadas ao conforto, portanto sĂŁo um foco de atenção importante para os PS. Todo o percurso desenvolvido Ă© pautado pela responsabilidade e vontade em disseminar a filosofia e as boas prĂĄticas na ĂĄrea, procurando constituir um fator motivacional para outros profissionais.
Abstract: This report appears as an integral part of the requirements necessary to obtain the Master's degree in Palliative Care (CP). It is a reflective process related to clinical practice developed with a differentiated team in PC, and is guided by the acquisition of skills. In a descriptive and thoughtful way, this report is supported by scientific evidence, describes and explains the activities developed and the intended objectives. Generally, it is organized into three moments: The clinical practice developed, organized according to the pillars of palliative care, indicating the skills in the implementation of a quality care plan for the person with chronic, advanced and progressive disease and their family, maximizing the quality of life and reducing suffering; presenting the methodology for acquiring instrumental, interpersonal and systemic skills. The intervention project implemented, with a focus on the clinical diagnosis of Last Days or Hours of Life (UDHV) which consisted of the elaboration of a training plan, presented to UCSP, where I develop professional activity, training professionals to develop basic actions palliative care, identification and early referral of the patient with palliative needs, particularly for the UDHV period. And, with the aim of deepening knowledge in this vast thematic area, the development of the Systematic Literature Review (RSL) - "Benefits of the early identification of the UDHV phase for the patient and his family", with the main objective of identifying the main consequences in the terminally ill patient and their family, which result from the early diagnosis of the diagnosis of UDHV by the Health Professionals (HP). The UDHV constitute a period of life, in which death is imminent, and it is necessary to redefine goals with the patient and their family, requiring at this stage, specific and adequate care to the person's needs, aimed at comfort, therefore, they are a focus of important attention to the PS. The entire course developed is guided by responsibility and willingness to disseminate philosophy and good practices in the area, seeking to constitute a motivational factor for other professionals.
